
South Africa’s digital healthcare transformation is moving from policy promises to on‑the‑ground projects, as the nation works to modernize patient records, expand telemedicine and test artificial intelligence tools in clinics.
National strategy drives early steps
The National Digital Health Strategy South Africa 2019–2024 sets out a plan to link health information systems, strengthen data platforms and increase the use of health data for decision‑making. The document aims to create a more connected environment where digital tools can improve patient management and service delivery.
COVID‑19 accelerated adoption of remote consultations and digital communication, putting pressure on traditional models. While providers added telehealth options, the pandemic also highlighted gaps in readiness, with many patients lacking reliable internet or devices.
“South Africa has made good progress, but implementation remains the biggest challenge. Digital healthcare is not only about introducing technology — it is about ensuring healthcare professionals can use it, patients can access it and infrastructure supports it,” said Mehnaaz Olla, School Manager of the MANCOSA School of Healthcare.
Connectivity and digital skills pose hurdles
Access to reliable internet remains uneven. The International Telecommunication Union reports that billions worldwide stay offline, with low‑ and middle‑income nations most affected. In South Africa, limited broadband coverage and affordability issues mean that remote‑area patients cannot always benefit from telemedicine.
Olla warned, “If we talk about digital healthcare without addressing digital poverty, we risk creating systems where technology improves access for some while leaving others behind.” The statement notes that investment in infrastructure must go hand‑in‑hand with programs that boost digital literacy.

One practical step could involve expanding community Wi‑Fi hubs and offering low‑cost devices through public‑private partnerships. Such measures would help bridge the gap between urban centers and rural clinics, allowing more people to connect to health platforms.
Workforce readiness and emerging technologies
Healthcare workers now need competence in electronic medical records, telehealth platforms and data management. “Digital competence is becoming as important as clinical competence,” Olla added, noting that new graduates will encounter these tools daily.
Artificial intelligence is being explored for medical imaging, decision support and administrative tasks.
Beyond training, research institutions are positioned to develop locally relevant AI models that reflect South Africa’s disease patterns and resource constraints.
Overall, the momentum will hinge on coordinated effort.
